LPO Status Change Form

 

Use the LPO Status Change Form for the following status changes:

  • Active to Inactive
  • Active to Voluntary Cancellation
  • Inactive to Active *
  • Inactive to Voluntary Cancellation
  • Suspension to Voluntary Cancellation
  • Change of Employer *
  • Name Change
  • Address Change

To reinstate your license from suspension, you must contact the WSBA directly.

Status changes are not complete until confirmed in writing by the WSBA. A new license will be provided for LPOs on active status with changes to name, employer, business address, and for a return to active status.

 

*For New Employer and/or Return to Active Status

 In addition to the status change form, when changing employers or returning to active status, the LPO must also provide proof of financial responsibility.

When using an errors and omissions or professional liability insurance policy, provide a copy of the certificate or declaration of coverage together with the Declaration of Financial Responsibility.

When using the employer's net worth based on the audited financial statement, complete and return the Certificate of Financial Responsibility in Lieu of Errors and Omissions Insurance.

 

Acceptable means for providing financial responsibility

An INDIVIDUAL ERRORS AND OMISSIONS insurance policy in the amount of $100,000, or an INDIVIDUAL AUDITED FINANCIAL STATEMENT showing the LPO’s net worth to be at least $200,000;

or

An existing AGENCY ERRORS AND OMISSIONS insurance policy in the amount of $100,000 which does not exclude the practice of law and which names you as a covered party;

or

A CERTIFICATE OF FINANCIAL RESPONSIBILITY IN LIEU OF ERRORS AND OMISSIONS INSURANCE form signed by an officer of your company accompanied by your employer's audited financial statement indicating a net worth in the following amounts and subject to Limited Practice Board approval:

  Net Worth      Number of Certified Employees

  $200,000      Each certified employee to and including five

  $100,000      Initial $200,000 plus additional $100,000 for each certified employee over five
