LPO Status Change Form

With the amendments to the Admission and Practice Rules (APR) effective Sept. 1, 2017, there are new requirements and forms for status changes. Please contact statuschanges@wsba.org or call 206-239-2131 for help with changing your status.

If you are active and changing your employer only, please send that information with your new financial responsibility information and documents to LPO@wsba.org.

Status changes are not complete until confirmed in writing by the WSBA.

For New Employer and Return to Active Status

 In addition to the status change form, when changing employers or returning to active status, the LPO must also provide proof of financial responsibility.

When using an errors and omissions or professional liability insurance policy, provide a copy of the certificate or declaration of coverage together with the Declaration of Financial Responsibility.

When using the employer's net worth based on the audited financial statement, complete and return the Certificate of Financial Responsibility in Lieu of Errors and Omissions Insurance.

Acceptable means for providing financial responsibility

An individual errors and omissions insurance policy in the amount of $100,000, or an individual audited financial statement showing the LPO’s net worth to be at least $200,000;

or

An existing agency errors and omissions insurance policy in the amount of $100,000 which does not exclude the practice of law and which names you as a covered party;

or

A certificate of financial responsibility in lieu of errors and omissions insurance form signed by an officer of your company accompanied by your employer's audited financial statement indicating a net worth in the following amounts and subject to Limited Practice Board approval:

  Net Worth Number of Certified Employees

  $200,000 Each certified employee up to and including five

  $100,000 Initial $200,000 plus additional $100,000 for each certified employee over five
