1. Download the Expense Report Form (PDF format). 
                         

2. Print and complete the form according to the directions. 

3. Mail to:
   Washington State Bar Association
   1325 Fourth Ave, Suite 600
   Seattle, WA 98101-2539

4. PLEASE NOTE: This form will be updated whenever mileage rates change. Please obtain the most recent copy to ensure correct expense reimbursement.
