Vital Statistics Forms in Family Law Cases

Due to strict enforcement specifications in place by the State Department of Public Health, effective immediately the King County Superior Clerk’s Office can accept only an original State of Washington Department of Health Vital Statistics form DOH-110-005 (REV 2/98) when initiating a dissolution or legal separation action. An original form is one that is printed and supplied by the Department of Health and is the only blank form that can be used. (RCW 43.70.160)
