LPO Status Change Form
With the amendments to the Washington Supreme Court Admission and Practice Rules (APR) effective Sept. 1, 2017, there are new requirements and forms for status changes. Please contact email@example.com or call 206-239-2131 for help with changing your status.
Status changes are not complete until confirmed in writing by the WSBA. If you need to simply update your name, address or other contact information, please go to the Member Tools and Information page.
For a New Employer
If you are active and needing to submit new employer information, please use the Notification of Change of Employer form and provide proof of financial responsibility.
When using an errors and omissions or professional liability insurance policy, provide a copy of the certificate or declaration of coverage together with the Declaration of Financial Responsibility Coverage form.
When using the employer's net worth based on the audited financial statement, complete and return the Certification of Financial Responsibility in Lieu of Errors & Omissions form.
Providing Financial Responsibility
An individual errors and omissions insurance policy in the amount of $100,000, or an individual audited financial statement showing the LPO’s net worth to be at least $200,000;
An existing agency errors and omissions insurance policy in the amount of $100,000 which does not exclude the practice of law and which names you as a covered party;
A certificate of financial responsibility in lieu of errors and omissions insurance form signed by an officer of your company accompanied by your employer's audited financial statement indicating a net worth in the following amounts and subject to Limited Practice Board approval:
Net Worth Number of Certified Employees
$200,000 Each certified employee up to and including five
$100,000 Initial $200,000 plus $100,000 for each certified employee over five.
Proof of indemnification by your government employer.
